Atypical epithelial hyperplasia of the breast: state of the art.

International audience; Introduction: Atypical epithelial hyperplasia (AEH) of the breast is considered benign histological lesions with breast cancer risk. This review focuses on clinical signification and management of AEH that remains controversial. Areas covered: A review of published studies was performed using medline database. Abstract Background Breast cancer is the most frequent malignant tumour to metastasize into the gastrointestinal tract in female and is second only to malignant melanoma. Nevertheless gastrointestinal metastases arising from breast cancer are quite rare. Underestimation of atypical lobular hyperplasia and lobular carcinoma in situ at stereotaxic 11-gauge vacuum-assisted breast biopsy

The aims of this study are to determine the frequency of diagnosis of atypical lobular hyperplasia (ALH) and lobular carcinoma in situ (LCIS) at stereotaxic-guided 11-gauge vacuum-assisted breast biopsy (VABB) and to assess the rate of underestimation of these at subsequent surgical excision and follow-up. Moreover, we aimed to define clinical, radiological and histological features of nonpalpable lesions in core biopsies that predict the lesion upgrade. Cytokeratin 8 immunostaining pattern and E-cadherin expression distinguish lobular from ductal breast carcinoma.

Immunohistochemistry using antibodies to cytokeratin 8 can serve as a valuable diagnostic tool for the differentiation of lobular from ductal carcinomas of the breast. In contrast with ductal carcinomas, which exhibit a peripheral-predominant immunostaining pattern, adjacent tumor cells “molding” to each other, lobular carcinomas exhibit a ring-like perinuclear immunostaining pattern, creating a “bag of marbles” appearance with neighboring tumor cells. Primary triple-negative invasive lobular breast carcinomas (TN-ILCs), which do not express hormone receptors and HER2 at diagnosis, are rare and poorly known. Antisera to CEA were used for the immunohistochemical localization and quantification of this antigen in 120 Bouin-fixed, paraffin embedded mammary carcinomas. These results were compared to tumor type, grading, staging, biochemical receptor status, cytosolic CEA-levels of the same tumors, and preoperative plasma CEA-levels. Mammary carcinomas were usually characterized by a low percentage of CEA-positive tumor cells: 50.9% of the cases contained more than 5% CEA-positive tumor cells and were therefore defined as being CEA-histopositive in this study. GPCALMA: An Italian mammographic database of digitized images for research

In this work the implementation of a database of digitized mammograms is described. The digitized images were collected since 1999 by a community of physicists in collaboration with radiologists in several Italian hospitals, as a first step in order to develop and implement a Computer Aided Detection (CAD) system. 3369 mammograms were collected from 967 patients; they were classified according to the type and the morphology of the lesions, the type of the breast tissue and the type of pathologies.
